Welcome to the Middle School Matters Institute (MSMI) Tier II Online Application

We are now accepting applications from middle grade schools to receive Tier II support from MSMI. A full description of Tier II support, details about the 2014 MSMI Summer Conference, and a description of the application process can be found in this downloadable flyer.

All applications are due at 5:00 PM Central Time on February 10, 2014.

Please complete all items to be considered for participation. Note that some questions require narrative responses. Fields with an asterisk are required.

This application consists of two parts. After you complete Part 1 (the questions on the next page), you will be directed to Part 2: the MSM School Readiness Assessment. If you have already completed the MSM School Readiness Assessment, you may include your results link rather than submitting a new readiness assessment.

Note that your Tier II Application is not complete until you submit both Part 1 and Part 2.

Tips for Submitting an Application

  • We recommend that you complete this application in either Firefox, Chrome, or Safari browsers. We strongly recommend against using Internet Explorer.
  • School staff may find it helpful to discuss application questions with their leadership team and gather necessary data before completing the application online. If you would like to plan your responses, you may download a copy of the questions here.
  • The application page is set to time out after 2 hours. It is highly recommended that users either open the application when they are ready to begin or refresh the page before supplying any responses. If the application page is open for longer than 2 hours you will not be able to submit any responses.
  • The application asks about content dimensions that the applicant would like assistance with. For more information about the content dimensions, please see the Research Platform page and the MSM Field Guide.
